Dafodil Hospital, its doctors, nurses, and entire staff are committed to providing you with excellent care, both clinical and personal. In addition, it is our policy to respect your individuality and dignity.

PATIENTS RIGHTS

We support your right to know about your health and illness and also the right to participate in the critical/vital decisions that affect your well-being.

1. Right to proper medical care:

You have the right to receive the best care medically indicated for your problem regardless of race, religion, national origin, or the source of payment for your care. You have the right to prompt life-saving treatment in an emergency without discrimination on account of economic status or source of payment.

2. Right to be treated respectfully:

You have the right to be treated respectfully by all employees of Dafodil Hospital. Your individuality will be respected and differences in the cultural and educational background taken into account. When you have a question, you will be listened to and receive an appropriate and helpful response.

3. Right to privacy:

You have the right to privacy. You may expect to talk with your doctor, nurse, or our administration representative in private, and know that the information you give will not be overheard or given to others without your permission. In the hospital when you are in a semi-private room you can expect a sincere and reasonable attempt to keep all conversations confidential.

When you are being examined you are entitled to privacy. If you are hospitalized, no outsiders may see you without your permission.

Your hospital records are private as well and no person or agency beyond those caring for you may be permitted to see those records except the authorized statutory bodies.

4. Right to medical-related information:

You have the right to seek and receive all the information necessary for you to understand your medical condition. You have the right to know the name of the doctor who is responsible for your care and to talk with that doctor and any other who give you care. You are entitled to know the probable diagnosis, planned course of treatment and the prognosis for the future. You have the right to ask your doctor or nurse any question that concerns you about your health.

5. Right for a second opinion:

You have the right to select your doctor and your treatment. You have the right to say “yes” to the recommended treatment. You also have the right to say “No” to treatment and ask for medical consultation or a second opinion on your medical condition, if you so desire.

6. Right to leave the Hospital:

You have the right to leave the hospital even if your doctor advises against it. If you decide to leave before the doctor’s advice, the hospital will not be responsible for any harm that this may cause you and you will be asked to sign a “Discharge Against Medical Advice” form.

7. Right to the cost of treatment:

You have the right to understand and receive information on the payment of hospital bills on a day-to-day basis in accordance with the policies of the hospital.

You and your designated relative have the right to factual information regarding the expected cost of treatment based on evidence. You will be given this information upon request. You can see your detailed bill from the kiosk at reception. You will be informed about any additional cost to be incurred due to a change in the physical condition of the patient or line of treatment. On completion of treatment, you have the right to receive a final bill, an explanation for the bill(s) regardless of the source of payment or the mode of payment, and receive the payment receipt(s) for any payment made.

8. Right to be free from any restraint or seclusion:

You have the right to be free from any form of restraint or seclusion that is not medically necessary or used as means of coercion, discipline, convenience, or retaliation by staff.

Restraint or seclusion may only be used to improve your well-being or protect you from harm when you or your family members have been informed and given approval by written consent.

9. Right to informed consent:

No medical treatment shall be administered until you or designated members from your family have consented to it. Such consent will be ‘informed’, based on the diagnosis, the nature of the proposed treatment, the risk involved (including pain and discomfort), and the chances and risks of alternative treatment of lack of any treatment at all. Your doctor will furnish the information to you at the earliest stage of the treatment in a manner that maximizes your ability to understand the information and to make a free and independent decision.

10. Care under emergency:

In case you are in a medical emergency, you will be eligible unconditionally to receive medical treatment. The doctor, to whom you turn up or are referred, is obliged to examine and treat you to the best of his/her ability. If the doctor is unable to do so, he/she shall refer you to a facility that can provide the medical care which suits your requirements. In case of any life or limb-saving situation, our doctor(s) will go ahead with the required emergency management, even against your will as per our hospital policy.

11. Right to complaint:

You, your family, or your guardian has the right to tell us when something is wrong in the delivery of your care. If you do complain, your care will not be affected in any way. Your feedback is welcome at all times.

12. Right to be protected from physical abuse or neglect:

You have the right to be protected against falling from the bed/trolley due to negligence, any kind of assault, repeated internal examinations, and manhandling. You have the right to receive special precautions, especially at vulnerable stages like the elderly, neonates, etc.

13. Right to have his/her information confidential:

You have the right to have your information treated as confidential. Staff should avoid any patient-related discussion in public places. Your information shall be revealed with respect to privileged communication. Your serology status should not be revealed or written on the cover of the medical records without your permission.

14. Right to give informed consent before transfusion of blood and blood products, anesthesia, surgery, initiation of any research protocol, and any other invasive/high-risk procedures/treatment.

You have the right to give informed consent for any elective procedure like a transfusion of blood and blood products, surgery, anesthesia, and any invasive procedure. You should provide informed consent for participation in research activities. You also have the right to withdraw from the research activities.

15. Right to access his/her clinical records:

Patients are entitled to receive information concerning your treatment including and copy of your medical record from your doctor or from the hospital. In case, where such information may cause serious harm to your health or endanger your life, your doctor may decline to give you such information.

16. Right to get information on the care plan, progress, and information on their health care needs.

You have the right to get information about their treatment plan, the progress of the treatment, and or intervention. You have the right to be counseled by the doctor and also informed about your healthcare needs.

 

17. Right to have special preferences, spiritual and cultural needs being respected.

You have the right to tell us any dietary preferences or any worship requirements. The same shall be respected and taken care of based on the hospital policy.

18. Right to proper referral and transfer, which is free from perverse commercial influences.

The hospital management will ensure proper referral and transfer of patients regarding such a shift in care. The referral process will not be influenced by any commercial consideration.

19. Right to choose alternative treatment options if available.

 

20. Right to safety and quality care according to standards.

 

21. Right to non-discrimination:

You have the right to receive treatment without any discrimination based on your illnesses or conditions.

22. Right to transparency in rates and care according to prescribed rates

23. Right to know the identity of the care provider

24. Right to protection for patients involved in clinical trials:

If you are approached to participate in a clinical trial, then you have the right to seek due protection in this context.

25. Right to take the discharge of the patient, or receive the body of the deceased from the hospital.

26. Right to Patient Education:

You have the right to receive education about the major facts relevant to your condition and healthy living practices, your rights and responsibilities, and officially supported health insurance schemes relevant to you.

27. Right to be heard and seek redressal:

You and your relatives have the right to give feedback, make comments, or lodge complaints about the health care they are receiving or have received from a doctor or hospital. You and your relatives have the right to a fair and prompt redressal of their grievances.

PATIENTS RESPONSIBILITY:

1. Be honest with us; tell us all you know about your present illness, your previous hospitalizations if any, and any other matter relating to your health that will help us treat you better.

2. Please tell us if you do not understand anything concerning your care or if you are unable to follow any instructions.

3. Respect the right of other patients and staff of the hospital. Please ensure that your visitors are considerate of other patients particularly with regard to noise and observe the “visiting hours” schedule of the hospital. Also abide to the visitor’s policy.

4. Observe the “No Smoking” and other rules of the hospital and co-operate with hospital staff in creating a safe and secure environment.

5. You have a responsibility to follow the treatment plan established by your physician, nurse and the healthcare professionals.

6. Be on time for appointments and inform the hospital if you cannot keep your appointment.

7.  Hospital staff has demanding jobs to do, often under stressful circumstances. Help them by treating them considerately. Violence or racial, sexual or verbal abuse is completely unacceptable. Please follow all advices given to you by your caregivers. If you are worried or have any doubt about doing this, discuss it with the person giving you the advice at that time itself.

8. Important services are only successful, if the people providing them know what you think about the services. Help staff by filling the feedback forms if they approach you.

9. Use emergency services in a real emergency only. Do not forget that there will be seriously ill people who need to use these services.

10. You have the responsibility to be prompt with the payment of your hospital bills and wherever necessary providing information essential for processing your bills.

11. Do not prescribe medications prescribed to you to others.

12. Please cooperate in maintaining confidentiality of your information and personal privacy by abiding to the restrictions and protocols related to the release of information.

13. We do not encourage self-medication; please do not take any medication without the knowledge of your care provider.

14. Since we adhere to the policy of uniform care to the patients, please do not request for imparting any preferences.

15. Cooperate with the doctor during examination, diagnostic tests and treatment, and follow doctor’s advice while keeping in view your right to participate in decision making related to treatment.

16. Please communicate with healthcare provider if your condition worsens and does not follow the expected course.

17. Please accept, where applicable adaptations to the environment to ensure a safe and secure stay in the hospital.

18. Please accept the measures taken by the hospital to ensure personal privacy and confidentiality of medical records.

19. Please understand your rights properly and seek clarification if required.

20. Respect the dignity of the doctor and other hospital staff as human beings and as professionals. Whatever the grievance may be, you / relatives should not resort to violence in any form and damage or destroy any property of the hospital or the service provider.

21. You should take responsibility for your actions based on choices made regarding treatment options, and in case you refuse treatment.
